For Providers
Thank you for considering referral to HypermobileDr.com. We support patients with hypermobility, HSD, hEDS, and EDS. Within this focus, we help address overlapping symptoms and conditions that may include dizziness or racing heart, faintness, food or chemical reactions, chronic pain, fatigue, GI dysfunction, nutrition concerns, neurodivergence-related complexity, and nervous-system dysregulation.
What We Need to Begin a Referral
- Completed Provider Referral Form
- Patient name and contact information
- Referring provider name and contact information
- Main reason for referral
- Brief referral question or concern
What We Do Not Need Initially
Please do not send the patient’s full medical record with the initial referral. At the time of referral, we do not need full chart records, long clinical narratives, complete lab history, complete imaging reports, specialist records, insurance paperwork, or prior authorization documents. If more information is needed, we will request specific records or clarification after reviewing the referral.
